Ha, turns out the tournament I am prepping for is actually doing 1v1 EDH. Ack.
Also noticed I wasn't running the full 100. Must have missed some stuff in all of my edits, so some of the + cards probably should have already been in.

-1 Syphon Mind
-1 Mind's Eye
-1 Oblivion Stone
+1 Crucible of Worlds
+1 Staff of Domination
+1 Mind Twist

+1 Rout
+1 Nevinyrral's Disk

+1 Lightning Greaves
+1 Loxodon Warhammer
Kor Haven is included. Decided against Maze of Ith since it doesn't produce mana. Skullclamp seems cheaty. Why isn't it banned!?
Edit: Added fetchlands to enhance SDT's role in the deck.
Ah sorry, I swear I checked your list twice, guess I just didn't notice.
Anyway, some comments:
First, I think you should play Clamp...it's ridiculously powerful, especially in this deck. BW needs all the draw it can get, and this is the deck for it.
Next, I think you should drop the Thornbite Staff combo nonsense. It's too vulnerable in my opinion, epecially with all the mass sweepers everyone runs (including your own). A creature-based combo that requires an artifact that's goal is to sweep the board seems really redundant. Run Martial Coup and Decree of Pain instead, they're ripe with card advantage.
Eternal Dragon deserves a spot too. If you really want to keep the Darkness combo, Crovax seems like an easy cut. He's powerful, but Eternal Dragon gives you card advantage and late-game staying power that has more synergy with your general than Crovax does.

In the end, we will remember not the words of our enemies, but the silence of our friends.
I like what you're trying to do with Thornbite Staff, but I think you need to read it again. Killing the board with it takes mana...a lot of mana. It does work with Royal Assassin/Avatar of Woe, but that's the only combo with it you listed that works.
I do think you should run combos with Teysa though...that's most of the reason to run her over Ghost Council. Thornbite Staff doesn't work for the insta-kill, but Blasting Station and Altar of Dementia do work, and cost less mana than the Staff anyhow. Ashnod's Altar and Phyrexian Altar can also be used in this deck to produce infinite mana, which I'm sure you could find a use for. You should probably run at least the Phyrexian Altar.
I notice you're running Beacon of Immortality...I do too. I would add in False Cure. It's a 2 card, 8 mana, instant speed combo that just flat out kills someone. You should have enough tutors to make this worth the card slot.
Some other cards I run that you might want to consider: Springjack Shepherd Stonecloaker (sometimes you need the graveyard hate, lets you reuse citp creatures) Necrotic Sliver (Reveillarkable, versatile removal) Ajani Goldmane Academy Rector (easy to kill, gets you Debtor's Knell or an answer) Nether Traitor (makes infinite mana and infinite tokens with Teysa and Phyrexian Altar, no Painter's Servant required) Yosei, the Morning Star(with recursion and a sac outlet, wins games) Bitterblossom (yes, white tokens are better than black tokens in this deck. this is still too good to pass up.) Condemn (one of the better removal spells available) Helm of Possession (haven't tested this, but seems like it should be really good here)
I used to play this deck myself until our local store banned Painter's Servant, Enlightened Tutor, and Skullclamp :(, At which point there was little reason to try to push the issue with the Teysa combo. I miss this deck.
Your build looks to be pretty strong although I don't see Cenn's Enlistment which was actually pretty good for me. Also, no Desolation Angel? That card is the nastiest girl on the block and can win games by herself. Also, I found Decree of Pain to be quite good.
Royal Assassin is a bit weak, but if you want a similar effect I would include Deathbringer Liege as it does much of the same, if not better, but also pumps your tokens. And in the name of all things holy, please do yourself a favor and put a Bitterblossom in there, I know it produces black creatures, but this is far and away one of the best token generators around. If color is a concern there is always Eight-and-a-Half Tails to shore this up.
Also, why would I remove a good chunk of my artifact/enchantment destruction? (Akroma's Vengeance, Nevinyrral's Disk, Magus of the Disk).
I know I need artifacts/enchantments to pull off a combo, but they are cheap and easy to drop when I need them, and not before. Getting rid of problematic stuff on the other side of the table seems good.
I think your mass removal suite is fine as is, but if it were me I would probably opt for Decree of Pain and Catastrophe in place of O' Stone and Magus. Personally I think O' Stone is a bit overcosted and Magus is fragile.
Some of the cards you want to remove are really good, aka Rout. Consider this...
-1 Thornbite Staff
-1 Thran Dynamo
-1 Beacon of Unrest
-1 Magus of the Disk
-1 Final Judgment
-1 Crovax, Ascendant Hero
-1 Lightning Greaves
-1 Loxodon Warhammer
-1 Royal Assassin
-1 Avatar of Woe
I wouldn't cut Lightning Greaves. Protecting your general seems important in this deck if you want to chain your combos over multiple turns. Cut Dread Return instead. You'd rather sac the tokens to Teysa, and while a free reanimation spell is nice, you don't run that many creatures that can't already be returned with Karmic Guide and Reveillark. Leave the recursion to them.

Seriously? Condemn? 1:1 trades in a multiplayer format are really bad, and only seriously good ones (like Faith's Fetters, Angel of Despair, Vindicate...) survive to make the decklist.
I realize it ships a general to the bottom of a library. But how often would Condemn be the right call rather than a sweeper?

Condemn is amazing because of the fact that it puts away generals for a long, long time. It forces them to either tutor for it or pray they topdeck it. Taking away a general's ability to come back can entirely ruin a player's ability to compete when everyone else can keep re-casting their generals.

Skeletal Vampire is interesting, since every time I make a new set of bats I also net a white spirit with Teysa.
Speaking of bats, what about Belfry Spirit? Seems kinda janky since it makes black tokens while it itself is white, making it awkward with Teysa (and it removes itself with haunt, which is also awkward). I think Waylay is better.
O'stone can be dropped for a "rattlesnake," or just played and then popped for about the same cost as Rout (at instant speed), but also take out all non-land permanents. It can also randomly not destroy some of your own stuff. Seems good.

How do you play the Stone at instant speed? Do you mean cast it on your own turn and activate it at instant speed? Or did you mean you can somehow play it during someone's turn like Rout can?

In the end, we will remember not the words of our enemies, but the silence of our friends.
The difference is that people can play around the O'Stone and make you blow it before laying down additional threats. You can't really play around Rout (at instant speed that is). No one ever sees it coming.
What exactly do you want Yawg Will to accomplish? It seems clunky in a deck that doesn't have power or storm combo capability. I do realize, however, that you can go bonkers with Phyrexian Altar on the board (assuming you have the Teysa combo online) but is it really worth it?

You'll notice the newest version cut Yawgmoth's Will.
Mirror Entity seems weak. I can sacrifice my army to make my guys +X/+X! Oh, wait...
Speaking of armies, after some testing with the deck, this deck doesn't really make much of one. It is able to control the board rather well, and it is able to abuse Teysa's abilties, but it's token horde is usually pretty small (if usually self-sustaining).

You hit the nail on the head. I noticed the same trend when I was playing this deck, you will usually get enough guys to fuel the combo but your army isn't exactly overwhelming. I found that most of the time my actual WIN would come from something like Altar of Dementia or Mirror Entity.
Personally I think Mirror Entity is reasonably good, especially if you also run Rotlung Reanimator, though I'm not sure if you will. I happened to have a minor Cleric subtheme in my build so he was quite good.
Rotlung Reanimator + Mirror Entity + Teysa + sac outlet is kinda cool, but you'd have to continuously activate the Entity to make the new Teysa tokens clerics as well. I guess that is what Phyrexian Altar is for. Add Darkest Hour to make a huge army of zombies.
Staff of Domination is getting the nod into the MD. It fits the combo role and gives me something cool to do with infinite mana (like infinite life, keeping my opponent permanently tapped down, and drawing into one of my mana using token producers, which I can then make infinite guys with).
Also noticed I wasn't running the full 100. Must have missed some stuff in all of my edits, so some of the + cards probably should have already been in.

You should absolutely play Skullclamp. It is cheaty, but it's one of the best cards you could have.
I agree that you should run Eternal Dragon over Crovax, Ascendant Hero. Add Martial Coup for sure--it's the best wrath for this deck by a long shot.

You also want Enlightened Tutor (and maybe Yawgmoth's Will/Auriok Champion) for sure. You really don't want Nekrataal--consider that your best plays involve Darkest Hour/Painter's Servant on black.
I know it's hard to find space for all this...my Teysa deck isn't done yet because I'm stuck at about 115 cards, and am having trouble cutting any of them. I think you can afford to lose some cards though. I'd cut some of the colorless-mana producing artifacts (Thran Dynamo, Grim Monolith, Mana Vault), some reanimation (Dread Return, Beacon of Unrest), and some of your removal (Nevinyrral's Disk, Magus of the Disk, Akroma's Vengeance, Final Judgement, Royal Assassin, Avatar of Woe, maybe Rout). I'd also cut Crovax, Ascendant Hero, Lightning Greaves, and Loxodon Warhammer. See if that gets you anywhere. I'll keep working on my list and maybe post something in a day or so.
It does appear I read Thornbite Staff incorrectly. I just thought it worked like Goblin Sharpshooter. Doh!
I was semi-considering Ashnod's Alter and Nether Traitor already... I think they also need to go in the main deck.
