Well, I have an uncut (although destroyed) rare / mythic rare foil sheet (see my post for the gory details).
On the sheet, each of the 53 rares is printed twice and each of the 15 mythic rares are printed once for a total of 121. That comes out to about 1 mythic for every 7 rares.
I agree with one rare or mythic every box, but I have seen some that have 2. I have never seen a box that didn't have at least one, but
I think the big question is, how are foils distributed? Do they say, well every X packs gets a foil, then just throw in a random foil? Or every X pack gets a foil common, then every Y packs gets a foil uncommon, then every Z pack gets a foil rare, etc?
There is a preset sequence. For example, I dont have it with me to look up, but the Black Book showed an 80-pack sequence for M10 foils, rotating around to the beginning every 80 packs. Each sequence had 3 foil rare, 8 foil uncommon, 15 foil common, and 1 or 2 foil basic land, if memory serves. So every two boxes plus six packs or so, that's what you should get.

IIRC, the odds of opening a foil at all in an older booster pack (pre-Mirrodin at the latest) was 1 in 5 packs (and back then they liked printing Foil Basics more than Foil Commons).
Then there's the Foil Alara packs, in which you are guaranteed 1:1 ratios. [/Captain Obvious]
They print the rarity of foils on the boosters. Currently it's 1:67 cards, used to be something like 1:75 until a few years ago if memory serves right.
I'd assume the rarity distribution is the same as with normal cards. Everything else doesn't make sense.
You can actually find if a pack will have a foil in it or not by the color of that pack (a lot of the time they're darker than their non-foil pack counterpart).
People who play YGO do it a lot (the print company will tend to print a foil'ed pack on a different line than a non-foil'ed pack, therefore the pack will be a barely different color).
