In general, the early game consists of using the land-ramp creatures repeatedly, while hopefully milling yourself and getting juicy reanimation targets. There are enough answer creatures that you can deal with most things that the opponent throws at you, and eventually you can set yourself up for a massive Living Death in which you get 10-15 ETB triggers and run roughshod over any resistance that might be left.
Along with the Altar of Dementia (and perhaps Blasting Station), a good idea might be to run Reveillark for synergy with Karmic Guide. Here's the combo: You can start with either the Guide in the graveyard or the 'lark in the graveyard, the other has to be in your hand (Lark can be on the field when you have Blasting Station, Phyrexian Altar, or another sac outlet out). Let's say that Lark is in the yard and Guide is in your hand. You cast the Guide, with its ETB trigger targeting Lark. While that trigger is on the stack, you sac the Guide to your Station/Altar. Then the trigger resolves and Lark comes back in. You sacrifice that to your sac outlet, and its ability triggers, bringing back Karmic Guide, so you can start the whole thing all over again.
