It's used in some Eggs builds to tutor out Lotus Bloom. That's its only purpose as far as I know (to put a normally uncastable permanent onto the battlefield).
The mana cost is very restrictive (both color and cost wise), so what you're getting had better win you the game. I could see it being used as a land tutor (to, I don't know, grab Ghost Quarter against Tron when you have Surgical Extraction in hand), but Knight of the Reliquary already fills that role, and at instant speed no less.
Wargate is a pretty great card in EDH and has seen some construed play in eternal formats as well. Not sure what I would do with it personally but I do like the card.

It seems like a decent combo tutor. As someone said, you can find Lotus Bloom and that's enough to want to play it in some Eggs or Second Sunrise deck. The problem is, it's going to be a pain to cast a lot of the time. It costs X+Bant, so that's probably going to be a lot of mana for you for most combo cards I can think of.
Sadly, it's best for getting stuff where X = 0, and the only other nonbasic lands worth building around that are still Modern-legal are the Tron lands, which obviously don't like going into 3-coloured decks. Well, that and Sylvan Scrying and Expedition Map exist, and it would be preferred if Wargate could find awesome stuff where X doesn't equal 0 (or nonland cards where X = 0).
...Which means, sadly, Modern hasn't broken the card since Valakut got banned. It's found a tenuous home in Sunny Side Up (gets Lotus Bloom and Ghost Quarter), and that's it so far.
Agreed. EDH card. It's too janky to play in this format. You probably have to play Bant colors and I don't think too many card interactions can make it even slightly worthwhile.

Any Blooms you draw past the first turn are dead. Bloom feels like the Chancellors/Leylines in that regard.
There are more consistent options for ramping, such as the talismans, signets, and Pentad Prism (which Ideal uses). Storage lands (Calciform Pools) work too, especially in decks where you can keep counter mana up.
I'm going to have to agree with the choir here - this just can't do enough for the mana cost. It's a nice effect, but Chord of Calling has not being three colors, instant speed, and convoke in its favor. Lotus Bloom is probably the best thing to get with the card, and while it's not completely unplayable, not being able to get Prismatic Omen for Valakut kills its most effective use by far.
Rating: 1.5/5, because it's potentially good. Lack of instant speed really hurts this.
