When Nantuko Horror dies, put any number of target cards from your graveyard on top of your library, then each opponent puts that many cards from the top of his or her library into his or her graveyard.
I'd say that this card could probably be a rare, and on top of it, you could cut it down to a {3/X, where X>3} that way it won't be as tough. Hell, it will still be good. You can put your tarmogoyf, your dark confident, your inquisition of kozilek, your remand... all on top, along with this guy, if he's a legal target, all while milling your opponent.
In actually fairly sure this should be rare. I had a blue/black card that did something similar when it dealt combat damage to an opponent, but dying is a much more powerful trigger, I believe. Also, I like the idea of making it target "other cards" as opposed to being able to target itself. It can still target other copies of itself but at least not the dying copy. That would help balance it.
Anyhow, cool card, I'd play this in a BUG deck any day.
I'd cost Unnamed Card X at BUG and Nantuko Horror at 5cmc, honestly.
Compare Veilborn Ghoul and Rot Farm Skeleton, two other self-replacing uncommon 4/1s recently in Standard. They both can't block, they both require an additional input to recur themselves, and Nantuko Horror has a bunch of additional utility on top of the self-replacement effect, which balances against the additional color requirement.
Dark Revenant is not a card I'd be happy to pay retail price for, which is why the self-recurring clause is just as much a hindrance as a help. BUG is a perfectly fine cost to pay for a 4/1 vanilla creature. EDIT: Okay, probably a 4/2, considering Regathan Firecat. But I still wouldn't cost a vanilla 4/1 at 2cmc.
I agree that the self-recur aspect isn't actually gamebreaking, since it costs you a draw to get it back which will set you pretty far back if your plan is to repeatedly chump. I'm wondering if it should say "other target cards" for another reason though, which is that when I first read it I wasn't entirely sure if it could return itself or not. It clearly can, but that's a potential argument between players that could be avoided for little cost. It's a great BUG wedge card though, I think it's a good unique effect for that kind of deck.
